Getting everything ready for your return home
Many people can’t leave the hospital without some home changes or support. We will get your home set for your return, so you can leave the hospital as soon as you’re ready. This might include:

Is your hospital discharge support really free?
Yes! The NHS covers the cost of reablement support. If you need longer-term support, other funded care may be available to you. Our team are on hand to look at these possibilities with you.
What if I need support for longer than 6 weeks?
At Humans, we will never leave someone without the care and support they need. If you need support for longer, we will work with you to arrange a solution. We will ensure your longer-term needs are met. The direction we take will be unique to you and your circumstances.
