We make sure you are fully supported when you return home and ongoing. If you need support following your discharge, we will arrange this for you. And if you need long-term support, we’ll help with that too.

Getting everything ready for your return home

Many people can’t leave the hospital without some home changes or support. We will get your home set for your return, so you can leave the hospital as soon as you’re ready. This might include:

  • Buying essential items

  • Moving furniture for accessibility

  • Cleaning your home

  • Arranging support for when you get home

Supporting you during your recovery

We can support you for up to six weeks following your discharge from hospital. Our team will help you find areas where you might need support. Then, we’ll set up that support in a way that works best for you.
