Cornwall Carers Service (Carers Advice Line)

What it does: Provides advice and support to paid and unpaid carers in Cornwall.
Our role: Humans delivers the Carers Advice Line, offering practical information, guidance, and emotional support for carers.
Who else is involved: CRCC, Barnardo’s, Promas, and Age UK.

Inclusion Matters

What it does: Helps residents of Cornwall find information and advice to improve wellbeing, stay healthy, get active, and feel connected to their community.
Our role: Humans is a delivery partner, providing advice and support as part of the Inclusion Matters service.
Who else is involved: CRCC, Disability Cornwall, Age UK, Inclusion Cornwall, and Cornwall Council.

Home Together Collaborative

What it does: Supports people to return home safely after a hospital stay and through their reablement period.
Our role: Humans provides advice and support for people leaving hospital as part of the wider collaborative.
Who else is involved: Disability Cornwall, CHAOS, STEPS, Home First, and Age UK.

Dementia Together

What it does: Offers information and support for anyone in Cornwall living with dementia or worried about their memory.
Our role: What is our role?
Who else is involved: Alzheimer’s Society, Memory Matters, and Disability Cornwall.

Cornwall Disability Alliance

What it does: Brings together disability organisations across Cornwall to strengthen voices, influence decision-making, and improve services.
Our role: Humans facilitates the Alliance, coordinating activity and supporting shared projects.
Who else is involved: Active 8, Bridges for Change, Cornwall Memory Cafe, Cornwall Mobility, disAbility Conrwall and Isles of Scilly, Hearing Loss Cornwall, isight Cornwall, Merlin, and Mid Cornwall Lifestyles.

Moment Makers

What it does: Offer young people with learning disabilities the opportunity to volunteer at the memory cafe's around Cornwall.

What it does:Empowers and amplifies the voices of young adults with Learning Disabilities.

Projects

Current project we are part of include…

RCHT Accessibility Advisory Group

What it does: Co-designs infrastructure, refurbishment, and access plans across Royal Cornwall Hospital in Truro, West Cornwall Hospital in Penzance, and St Michael’s Hospital in Hayle.

Our role: Humans participates in the group as part of the CDA partnership. 

Who else is involved: All Cornwall Disability Alliance partners.

CDA Advocacy and Participation Project

What it does: Ensures people who are often underrepresented have their voices heard, leading to more tailored and effective services.

Our role: Humans helped secure dedicated staffing and continue to support delivery through the CDA.

Who else is involved: All Cornwall Disability Alliance partners.
